We are looking for a health professional with previous experience working with older people with disabilities for this permanent part time position working 48 hours a fortnight (0.6 FTE).
You will be working in consultation with a wide variety of community services, health services and provider agencies to support older people with disability needs to maximise their independence and encourage participation in the community.
