We are seeking two experienced, resourceful and resilient Mental Health Nurses for these key positions.
The roles will assist with the facilitation of mental health care in primary care settings through the provision of mental health assessment and clinical interventions.
The roles encompass:
- Co-ordination of primary mental health assessment and referral services (To Be Heard programme)
- Ongoing monitoring / clinical interventionsfor people with stable enduring mental health needs who transition back to general practice care
- The provision of mental health assessment, treatment and referral or consultation with specialist mental health service
